About Eyelid Surgery Procedures

Eyelid surgery, clinically known as blepharoplasty, is a cosmetic procedure that removes excess skin and fat from the upper and lower eyelids. Excess skin on the upper eyelids can make you look tired and older. For the lower eyelids, some patients may have large bags under their eyes, which can add to their aged appearance.

Dr. Bajaj offers upper and lower blepharoplasty procedures to help correct inevitable signs of aging around the eyes. Depending on the condition of your eyelids, Dr. Bajaj may recommend combining a blepharoplasty with a brow lift to raise your eyebrows for a more significant lift to the upper eyelids. For patients who have hollowness under their eyes, she may also recommend a dermal filler or facial fat transfer. Through a consultation, she can make the right recommendation for you.

Dr. Bajaj offers three types of blepharoplasty surgeries.

Upper Blepharoplasty Procedure

Upper blepharoplasty addresses concerns such as “eyelid droopiness”, excess skin, fine lines, and puffiness of the upper eyelids. Excess upper eyelid skin can cause you to appear tired, can make applying eye makeup difficult, and can also impair full range of vision.

An upper blepharoplasty procedure removes the excess skin and fat through an incision made in the crease of the upper lid. The incision is then closed with thin sutures.

Lower Blepharoplasty Procedure

Lower blepharoplasty is a procedure that removes excess skin and fat from the lower eyelids. It’s performed by making a small incision on the lower eyelid lash line. Through the incision, fat is either removed or the lash line is redraped. This smooths the appearance of the lower eyelid. Excess skin is removed if necessary to diminish wrinkles and lines that have formed below the eyes.

Combination Blepharoplasty

Eyelid Surgery Consultation

Dr. Bajaj offers eyelid surgery consultation to prospective patients. During your one-on-one consultation with Dr. Bajaj, she will ask you what your concerns are and what you are looking to achieve by undergoing a blepharoplasty procedure. She will then review your medical history, taking into consideration your current health, routine medications, and prior surgeries to help determine if you are a good candidate.

A physical evaluation of your eyelids is necessary before surgery. Dr. Bajaj will look at your eyes and the surrounding skin to ensure that this treatment will be right for you. Depending on your skin’s current condition, you may require additional treatments or procedures in conjunction with the eyelid procedure. These will be discussed during the consultation.

What cosmetic issues can eyelid surgery address?

Eyelid surgery can address the hooding of the brows that occurs with aging, the excess skin of the upper eyelids, and the ‘bags’ that we can see in our lower eyelids.

An upper blepharoplasty will have a scar in your upper eyelid crease; a lower eyelid blepharoplasty will have a scar just under your eyelashes extending to one of the wrinkles in your crow’s feet.

On the day of your surgery, you will meet with me, and we will go over the surgery again. Then, one of my nurses will take you to the operating room where you we will start an IV and sedate you. The surgery will take 1-2 hours, after which you will be able to go home.

It will take about 7-10 days for the bruising and swelling to resolve. We will remove the sutures after 5 days. If you have a desk job, you should be able to return to work after 7 days.

The procedure is typically performed under general anesthesia or sedation.

Surgery will take between 1-2 hours depending on exactly what procedures you elect to have (upper eyelid blepharoplasty, lower eyelid blepharoplasty, and/or brow lift).

No. Since we will be giving you medication to sedate you, you will need a responsible adult to drive you home.

We will remove your sutures 5-7 days after surgery. Most individuals feel comfortable returning to work by 7-10 days after surgery.

Yes, there is bruising and swelling after any surgery. It will take 10-14 days for most of the swelling and bruising to resolve.

Any type of eyelid surgery will aggravate your eyes. During the healing process, it is important that you keep your eyes well-lubricated with eye drops and ointment.

The purpose of a blepharoplasty or eyelid surgery is to improve the appearance of the eyelids by removing the excess skin of the upper eyelids and improving the appearance of the lower eyelid bags and hollows – all of which combine to make us look tired.

The wrinkles around the eyes (crow’s feet) are best treated with a Botox and/or skin resurfacing procedure, which can directly treat these wrinkles. Similarly, droopy eyebrows are best treated with a brow lift. A brow lift is frequently performed at the same time as a blepharoplasty – in this way, we can rejuvenate the upper face and give you a refreshed look.  The dark circles under your eyes may be due to two issues – one is skin pigment, and the other is the hollowing that occurs that affects the way that light reflects off of your lower eyelids. Surgery cannot treat the skin pigment issues; however, either fat grafting or re-draping of your lower eyelid fat can improve the hollowing, which will improve the way light reflects off of your lower eyelids.

The bags under your eyes are typically due to the protrusion of the fat around your eye. One of the best techniques to address this issue is to release the tissue restricting the fat and then re-drape it so that it can fill the hollow.

In women, the eyebrow is typically 1 cm above the bone. In men, it frequently sits right at the bone. If your eyebrow is below this level, then you will likely benefit from a brow lift in addition to a blepharoplasty. Also, sometimes you may notice that your brow feels heavy, particularly at the end of day, because you have worked your forehead muscles to keep your brows elevated so that you are better able to see. During your consultation, I will perform a physical exam and evaluate the position of your brow as well as your eyelids.

The aging process will affect each individual at different rates. Part of the reason that we may choose to have a blepharoplasty before a facelift is that we may become more aware of the excess skin and bags under eyes sooner. However, many patients will combine a facelift with a blepharoplasty. Regardless, I advise my patients to prioritize the issues that bother them. Once you have your priority list, we can discuss the best options to address your concerns.

A good candidate for blepharoplasty is someone who is in good health, can undergo an elective surgery, and has realistic expectations. If you have had recent Lasik surgery, you should wait 6 months to one year after this procedure before undergoing an upper and lower eyelid blepharoplasty.

You will be able to resume sports and exercise after 3 weeks.

You will be able to drive after 5 days and return to work within 5-7 days. However, you will not be able to work out or do strenuous physical activity for 3 weeks.

You will be able to take a shower within the first 24-48 hours after surgery. It will be two weeks before you are able to take a tub bath.

Since eyelid surgery can irritate your eyes, it may be up to 4-6 weeks before you will be able to wear your contacts.
